The Department

The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department. Under the direction of the Manager, Social Work Department, the following services are provided:

  • Social Work
  • Aboriginal Health

Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.

 

This role will work within the Cardiac Heart Failure service and will sit in the Trauma stream within the Social work Department at The Alfred.  This role will work across the Cardiac Heart Failure service and will work within designated units in the Trauma stream.   The successful applicant will need an understanding of the acute health setting and provide discharge planning and supportive care.  The role involves direct Care, including comprehensive Social work psychosocial assessments, counselling, adjustment to illness support and clinical intervention.

The Social Worker in CAHF and Heart Transplant is responsible for the provision of advanced social work services to patients and their families/significant others within the Alfred Heart Transplant and Cardiac Heart Failure Service.  The worker will be required to consult with colleagues and staff from other disciplines on relevant issues as required, and to provide advanced casework expertise.  The worker will be required to complete comprehensive pre-transplant assessments using an assessment tool - The Stanford Integrated Psychosocial Assessment for Transplant (SIPAT).  The worker will also work within the Trauma stream and will provide support to Social work within designated units allocated within the Trauma stream by the Trauma Team Leader.   

 

This position may be required to work at any or all campuses of the Alfred Health as and when required. Such a requirement may be ongoing, a temporary arrangement, on an ad hoc basis or a permanent change of location.

 

Skills/Qualifications Required 

Essential

  • Eligibility for membership of the Australian Association of Social Workers. Bachelor of Social Work, or recognised equivalent
  • Ability to work effectively in a multi-disciplinary team and with families, exhibiting initiative and facilitative qualities
  • Required to perform work that requires special knowledge and depth of experience in any one or more of the following:
    - Family Violence/ Child Protection/ NDIS
  • Knowledge of current Legislation and Policy in relation to Social Work in a hospital setting

Desirable

  • Working towards or Postgraduate qualifications in Social Work or a related field
  • Hospital-based work experience
  • Sound IT Skills
