The Alfred Renal Service combines both inpatient and day ambulatory services, providing specialist renal services including home therapies (Peritoneal Dialysis and Home Haemodialysis Dialysis), renal transplantation, acute and chronic dialysis and pre-dialysis coordination, including outreach and vascular coordination of renal patients. The service has a home before hospital focus which encourages patient independence and promotes healthy wellbeing.   

The Alfred Hospital Renal Transplant program began in 1966 and remains of the major kidney transplanting services in Victoria. The Alfred’s Renal Transplant program provides the following services: 

• Evaluation and preparation of kidney transplant recipients and live donors 
• Kidney transplantation (deceased donor, living donor, paired kidney exchange, pre-emptive transplantation) 
• Long term follow up care of kidney transplant recipients and live donors. 
• Dedicated pre/post transplantation out-patient clinic 


The Renal Transplant Coordinator will be responsible for providing professional support and clinical expertise to people with end stage renal disease who have chosen a kidney transplant care pathway. 

The position works closely with Renal Service stakeholders to achieve the coordination of transplant assessments and follow-up of patients with end stage kidney disease suited to kidney transplantation.  The Renal Transplant Coordinator is a clinical leader who actively contributes to meeting key organisational goals which are: 

• To coordinate the assessment and care of patients in both the pre-dialysis phase and the dialysis phase flagged for kidney transplantation work up, during transplantation and post transplantation. 
• To coordinate all aspects of kidney transplant care in relation to evaluation and work up for transplantation, including establishing appropriate timely work ups for live donor and deceased donor kidney transplantation. 
• Escalation of any variances is a critical component and requires coordination and facilitation of any issues that are impacting on equitable and/or timely access to kidney transplantation. 
• Key contributor to the ongoing development and quality improvement in all modalities of transplantation including especially live donor transplantation



• Current Registered Nurse Registration with Nursing and Midwifery Board of Australia via AHPRA.   
• Extensive relevant clinical experience in acute care nursing and in a clinical leadership role.   
• Excellent negotiation and interpersonal skills   
• Ability to work autonomously with efficient time management skills   
• Computer literacy – Microsoft Office, excel    
• Current and satisfactory Police Check   
• Current and satisfactory Working With Children Check   


• Min 5 years recent renal dialysis nursing experience   
• Previous experience in a similar role   
• Recent acute renal care or community nursing experience   
• Demonstrated experience in patient education programs and or adult learning program   
• Qualifications / Certificates – Post graduate qualifications (or working towards same) in Renal Nursing is highly desirable
