The Department
The Social Work Department operates in accordance with the philosophy and goals of The Alfred; a focus on the quality of life and participation in care is a primary goal of the Department. Social Work aims to identify and assist in addressing the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services, such as counselling support, grief and loss support, advocacy, service provision and complex and considered discharge planning.
The Position Summary
The successful applicant will have significant Social Work expertise and a demonstrated ability to work effectively with patients and families to provide counselling, support and comprehensive discharge planning. They will also be expected to contribute to the the ongoing development of the Social Work team in collaboration with the Team Leader within the Social Work Service. The successful candidate will have a strong knowledge base in relation to hospital Social Work supports and services and have a good understanding of dealing with an Elderly and Rehabilitation patient cohort. A sound knowledge of supports for carers and the ability to engage with care agencies and external stakeholders is imperative in supporting this patient cohort. Excellent interpersonal skills are essential for this position, including written and verbal communication, liaison and advocacy skills. An ability to display flexibility within work practice and to work as part of a multi-disciplinary team is also essential. A significant clinical load is essential in this position as well as providing supervision to staff. This role may also require the successful candidate to work across other streams or Alfred Health sites
Qualifications and Experience Required
The successful applicant will also need to have experience in counselling, with a particular emphasis on adjustment to illness and hospitalisation. An understanding of the psychosocial issues associated with patients and their families and the
support services available post-hospitalisation will also be advantageous for this role. The Grade 2 Social worker will need to know about VCAT, TCP and NDIS processes and be able to support discharge planning.
