The Department

HARP (Hospital Admission Risk Program) Complex Care is largely a community-based service within the Health Independence Program (HIP).  HARP sits within the Alfred Home Acute and Community (HAC) Program and reports through the Acute and Integrated Stream (AIC). HARP provides education and care coordination to improve the health literacy, well-being, and health independence of individuals living in the community. The service promotes patient flow and aims to reduce potentially preventable hospital readmissions.

The Role

The Allied Health Assistant is aligned to the Central Team, and therefore functions across the HARP service, supporting the geographic teams, senior clinicians and the primary care clinicians. The AHA works in a flexible and adaptive manner as the HARP program continues to evolve.

The position is responsible for the implementation of individual interventions and/or therapy goals, as delegated by primary clinicians. The AHA will work as part of the multidisciplinary team to provide services that include provision of delegated therapies, provision of appropriate equipment and aids as prescribed by Allied Health Professionals (AHPs), assistance with paperwork and referrals to community services and liaison with external stakeholders to obtain information to progress care. The AHA will also be responsible for administrative and clinical support functions, which assist with client management and contribute to the effectiveness and efficiency of the team. 

The position may involve working across any clinical area at any Bayside Health sites.

Qualifications and Experience Required

Essential

  • Completion of Certificate IV (Allied Health Assistance) from a registered training organisation, OR  Recognised equivalent qualification (as determined by the line manager):  
    • Higher qualification in a relevant health science discipline at Diploma or Associate Diploma level
    • Certificate level qualification in a relevant health-related field 
    • Overseas-trained Allied Health practitioner with a qualification at Diploma level or higher
    • Allied Health undergraduate student (in a relevant health science discipline) having successfully completed one year of study at minimum
  • Current Victorian Driver's License (unrestricted)

Desirable         

  • In depth knowledge of community services
  • Experience working for community based services
