DEPARTMENT:

The Heart Failure Service in the Department of Cardiology provides investigation and management of both inpatients and outpatients with acute and chronic heart failure. The programme is state wide, with the support of advanced heart failure options which may include device therapy, mechanical support and transplantation.  The Heart Failure Outpatient programme is designed for adult patients whose major problem is Heart Failure, either HFrEF or HFpEF.  The programme offers a multidisciplinary approach to care including nurse co-ordinators, allied health and cardiac rehabilitation programmes specifically designed for heart failure patients. 

POSITION:

Heart Centre is currently looking for a fulltime Heart Failure Coordinator. You will have a demonstrated cardiac clinical expertise that will enable you to plan, implement, coordinate and evaluate heart failure patients’ care in close collaboration with members of the Acute Heart Failure Service and multidisciplinary team. In this role, you will aim to maximise the therapeutic benefits of medical management of these patients.  These include implementing strategies to improve quality of life, cardiac function, and prevent or reduce the need for admission or readmission to hospital.  Advanced heart failure management skills, particularly with inpatients is essential. Outpatient heart failure nursing experience is preferred.  Current registration with the Nurses Board of Victoria as RN, Division 1. Post-graduate qualification preferred.

QUALIFICATIONS AND EXPERIENCE:

Essential

  • The role requires a detailed understanding of acute and chronic Heart Failure management.  Specialist clinical experience in the field and demonstrated ability to deliver responsive, evidence-based care is essential
  • The ability to liaise with Heart Failure Cardiologists, General Practitioners, HARP, HITH, Palliative Care, allied health, ward staff, rehab, and other health services
  • Current AHPRA registration with the Nurses Board of Victoria as RN, Division 1
  • Post-graduate qualification in the cardiac specialty
  • Current National Police History check

Desirable

  • Demonstrated high level of knowledge of services which support people within the cardiac/heart failure specialty
  • Exceptional communication and customer service skills
  • Demonstrated high level problem solving and planning ability
  • Experience in conflict management/negotiations
  • Act as a patient advocate to ensure patient participation in all care/service planning
