The Advance Care Planning (ACP) program at Alfred Health promotes every person’s right to autonomy, dignity and fully informed consent in relation to their current and future healthcare treatment. The program supports individuals through a process of planning for future health and personal care and is underpinned by the Medical Treatment and Planning Act 2016 (Vic).
The Alfred Health ACP Program is resourced with clinicians and administration support. The program provides an inpatient and outpatient service for any Alfred Health patient or anyone living in the Alfred Health catchment to assist them through the ACP process. The program provides education and training for staff and consumers. The staff-based education and training is focussed on increasing awareness of ACP, on how to support patients with planning for future healthcare decisions and how to recognise and respect ACP documents when required in the clinical setting. Consumer information sessions are also held to enable members of the community to understand ACP and how it might support them and their families. ACP is a developing area across Australia and requires innovation, evaluation and research to continue to establish best practice.
The Voluntary Assisted Dying legislation came into effect June 19th, 2019, and the ACP Manager was responsible for implementation of the act at Alfred Health. A Voluntary Assisted Dying Liaison Officer was employed within the ACP program to coordinate referrals.
The ACP Clinician will provide high quality ACP service to inpatients and outpatients referred to the ACP program. The ACP clinician will be responsible for assisting patients with ACP discussion and the completion of ACP documents. The role involves supporting clinical staff to have ACP conversations with their patients and providing training and mentoring to enable these discussions to occur. The ACP clinician will be expected to work across all three hospital sites and visit the local community health centres and other community facilities (eg residential care facilities) as required.
