• Permanent – ongoing • Full-Time – 80 hours per fortnight (Sunday – Thursday) • Classification code – SC13 • Alfred location • Staff benefits DEPARTMENT: The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department. Under the direction of the Manager, Social Work Department, the following services are provided: • Social Work • Pastoral Care • Aboriginal Health Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home. POSITION: We are seeking a Social worker to work within the General Medicine Social work team at The Alfred. The successful candidate will have relevant hospital experience and be able to work within a strong multidisciplinary team. The candidate will need to have a good understanding of working within an Electronic Medical Record. The candidate will need to exhibit a good understanding of social work theories and apply them to hospital practice. It will be essential to have a sound understanding of complex discharge planning within an acute hospital setting in particular an understanding of Transition Care Program, My Aged Care and the NDIS. The successful candidate will need excellent communication skills to collaborate with patients, families, other disciplines and community stakeholders. This is a fast-paced environment and requires timely and quality discharge planning and the successful candidate will need to work flexibly, creatively and collaboratively. This role requires working weekends from Sunday – Thursday and public holidays. QUALIFICATIONS AND EXPERIENCE: • Academic: Eligibility for membership of Australian Association of Social Workers. Bachelor of Social Work, or recognised equivalent. • Personal: Ability to work effectively in a multi-disciplinary team and with families, exhibiting initiative and facilitative qualities.
