The Hospital Admission Risk Program Manager is responsible for the management and leadership of the HARP services at Alfred Health. This includes accountability for the overall development, coordination and delivery of the HARP Complex Care services across the multiple sites of Alfred Health. This position is responsible for monitoring the achievement of clinical and non-clinical performance targets and ensuring the provision of high quality care which aims to improve the capacity for people with chronic and complex health and psychosocial conditions to manage their wellbeing.
We are seeking an experienced allied health or nursing clinician to provide leadership and management to the Hospital Admission Risk Program (HARP) team during the current manager's secondment. You will require experience in managing multidisciplinary teams and demonstrated ability to work in partnership across different organisations within the health sector.
QUALIFICATIONS AND EXPERIENCE:
Relevant Allied Health or Nursing qualifications.
Current registration with AHPRA if applicable.
Demonstrated experience in the management of multidisciplinary health care teams.
Highly developed change management skills
Excellent written and verbal communication skills.
Understanding of the health care sector including hospital, primary care and community services
Current Victorian Drivers Licence
