  • Fixed term part time until 26/06/2022
  • 64 hours per fortnight

Department

HITH provides acute nursing therapy to patients in the community as a substitution for care otherwise provided in the inpatient setting. Alfred Health HITH cares for a diverse cohort of patients, within metropolitan Melbourne and across the state. HITH operates across the Alfred and Sandringham Hospitals, providing a seven day a week service. Patients can be referred from The Alfred, Sandringham and Caulfield Hospitals.

The role

Alfred Health Hospital in the Home (HITH) is seeking applications from experienced nurses with an interest in coordinating and optimising acute clinical care in the community setting. HITH has a pivotal role in optimising hospital patient flow and access by delivering high quality, home-based acute care. The suitable applicant will become part of a supportive and friendly multidisciplinary team. The HITH clinical coordinator participates in a seven day per week rotating roster, with no night duty.The role supports home visit clinicians to deliver well-organised, safe and effective care for patients in their usual place of residence.The coordinator also assumes a variety of other roles within the service including patient access and intake, on boarding, goal setting, care planning, care coordination, and discharge planning.Alfred Health HITH provides a diverse range of clinical care and therapies, both locally and state-wide, in partnership with numerous programs and service providers.

  • Respiratory and Cystic Fibrosis
  • Surgical 
  • Gastro-enterology
  • Burns and Plastics
  • Oncology
  • Heamatology
  • Neurology
  • Cardiology
  • General Medicine
  • COVID-19

The HITH clinical coordinator works in partnership with Medical, Nursing, and  Allied Health clinicians.

The successful incumbent with have at least 4 years of current experience in the provision of acute clinical care. A developing or developed understanding of the complexity of providing acute care in the community setting is highly favourable.

A  commitment to service development and quality improvement in service delivery is essential.

The successful applicant will have excellent professional communication skills and be adept at liaising with multiple stakeholders, patients, and carers, and be committed to delivering high quality, timely, effective, and patient centred care.

Skills

  • Registered as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA.
  • Experience within the acute and or ambulatory/community setting desirable
  • A current Victorian Drivers Licence DESIRABLE KEY SELECTION CRITERIA
  • Post Graduate Qualification in relevant field
