Alfred Health

Alfred Health is a leader in health care delivery, improvement, research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne, from ambulatory, to inpatient and home and community based services.

  • Case manager, Alfred Health Home Care Packages
  • Allied Health Grade 2 or Registered Nurse Grade 3
  • Fixed term, maternity leave cover until November 2021
  • Full time - 80 hours per fortnight (with ADO)
  • Based on site at Caulfield Hospital, only 15 mins south-east of city

The Role

You will work within the supportive, flexible and dynamic Alfred Health Home care packages team, based at Caulfield Community Health Service.  Home care packages support older Australians to optimise their health and well-being in accordance with their needs and goals.  Packages deliver co-ordinated consumer-directed care and services to meet these goals.

You will be an experienced Grade 2 Allied Health Professional (Social Worker, Occupational Therapist, Physiotherapist) or Registered Nurse who will:

  • Be responsible for the provision of advanced case management for a caseload of complex and diverse adult and older clients
  • Provide effective and consumer-directed care planning and care co-ordination, with a wellness and reablement approach
  • Provide high quality services to clients from a variety of social, economic and cultural backgrounds
  • Collaborate effectively with interdisciplinary team members and service providers
  • Achieve monthly client target hours and key performance indicators
  • Effectively manage clients’ package budgets
  • Promote the Alfred Health Home Care Package program both within Alfred Health and our local community
  • Seek professional support and mentoring from Team Leaders, and discipline specific allied health professionals

Skills/ experience

  • Relevant Qualification- Allied Health discipline (Social Worker, Occupational Therapist, Physiotherapist) or Registered Nurse
  • Eligible for membership in discipline specific professional body
  • Current Victorian Driver's Licence
  • Extensive experience in health and aged care
  • Excellent communication skills (verbal and written), collaboration and problem-solving skills
  • Well-developed organisational and time management skills
  • Well-developed computer skills
