HARP Complex Care is a community-based service within the range of Health Independence Programs (HIP), and in Alfred Health sits within the Community Integration Program.
This role works within a multidisciplinary team as part of the HARP centralised service. The occupational therapist is responsible for conducting comprehensive patient assessments and developing and implementing patient-centered care plans in collaboration with the patient, family, carers and broader HARP service.
The position works in a dynamic context in a multidisciplinary model to provide an integrated flexible model of care. HARP works in collaboration with a multitude of stakeholders and the patient to improve the health and wellbeing of individuals through clinical and psychosocial interventions
The successful applicant will have:
Sound assessment skills, the ability to determine and implement plans of care in the community to improve patient safety and quality of life and be able to recognise risk and or deterioration in the community and escalate appropriately
A strong interest in caring for people who experience significant challenges in both chronic disease, vulnerability and psychosocial complexities
This role opens opportunities for the incumbent to gain valuable experience in the management and care of diverse, challenging and dynamic problems for people with chronic and complex health conditions.
