Alfred Health is a leader in health care delivery, improvement, research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne, from ambulatory, to inpatient and home and community based services.
Better at Home is an inpatient substitution program modelled on both a rehab and aged care ward. As an alternative to an inpatient subacute stay patients receive goal based care in their home environment.
We provide home-based assessment and treatment using a multi-disciplinary team approach that incorporates medical, allied health, pharmacy and nursing therapies.
The program aims to reduce time spent in hospital, prevent a patient re-admitting after discharge or completely avoid a hospital admission.
To be a successful part of the Better at home interdisciplinary team you will:
- Provide quality patient care in the home environment
- Evaluate and validate strategies to exceed patient outcomes
- Develop and maintain collaborative relationships with various other disciplines
- Support and participate in programs to ensure quality improvement
To be considered for this newly created role you will have:
- Significant proven occupational therapy or physiotherapy expertise, preferably in a home based environment
- Excellent interpersonal skills
- A key focus on contribution with service development and evaluation
As this role is based " on the road" you will be responsible for making clinical decisions regarding escalation of care so you will be a positive, confident and experienced allied health professional looking to be part of a successful team.
You MUST possess a full Victorian Drivers Licence and be flexible to work a variety of shifts including weekends.
This position is open to either an occupational therapist or a physiotherapist.
The role will be as a part time Grade 3 Occupational Therapist or Grade 3 Physiotherapist within Caulfield Hospital including Better at Home.
