The Alfred Health HITH unit is a leading provider of acute home based health care in Victoria. The unit currently manages and treats in excess of 50 patients daily.
This dynamic role involves collaborating with multidisciplinary teams across a wide range of medical and surgical specialities within Alfred Health, and with outsourced service providers.
Patients more frequently accessing HITH include Cystic Fibrosis, Transplant, Heart Failure, Burns and Haematology patients, making Alfred Health HITH both a challenging and rewarding place of employment.
As a Clinical Coordinator, your role is to accept, coordinate and manage Hospital in the Home patient admissions. You will operate under a care coordination model, liaising with in-patient parent medical units, specialist roles and community organisations to ensure both patient management and discharge planning is efficiently and effectively implemented.
You will have advanced clinical assessment, leadership and communication skills and the ability to problem solve. You will be motivated, flexible and patient centred in your approach.
