  • Full Time / Part Time

The Department

The Palliative Care Service provides a predominantly consultative service to patients of Alfred Health.  Team members include consultant medical staff, palliative care nurse coordinators, junior medical staff and a Grief Counsellor as well as administrative support.  We provide advice on pain and other symptom management, facilitate discharge planning and assist units with complex decision-making, particularly relating to end-of-life care. Referrals are received from Alfred Health campuses. 

The Role

An exciting opportunity has arisen to join our expanding palliative care consultancy service at Alfred Health.

As a Grade 4 CNC you will work across all Alfred Health campuses to provide palliative care expertise for both inpatients and outpatients. You will utilise your excellent communication skills, proficiency at carrying out a comprehensive assessment and ability to work with a variety of teams to deliver optimal patient outcomes, concentrating on complex care and decision making to improve the transition from acute to end of life care.

Skills

You will have:

  • Proven capacity to work independently as well as within a team setting
  • Strong capacity to influence and support change and a patient centred culture of service delivery
  • Excellent communication skills
  • The enthusiasm and motivation to educate staff
  • Clinical expertise to provide day to day service delivery whilst exploring new ways of working across the clinical teams, including advance care planning and early intervention for end of life care planning in chronic illness
  • Recent palliative care experience, preferably acute hospital setting required.
