A fantastic opportunity exists for an experienced Social worker to work in the Cardiac Heart Failure and Heart Transplant service. You will have demonstrated prior experience in managing a complex clinical caseload and will have sound knowledge and skills in grief and loss counselling.
You will possess strong assessment, case planning, report writing experience and have an understanding of discharge planning which will be essential to this role. You will work within a strong multidisciplinary team and will be part of an organisation where professionalism, innovation and the ability to find creative solutions to challenges is highly regarded.
A current and valid Working with Children’s Check will be required to work within the core days in which the Cardiac Heart Failure and Transplant Team.
