HIV Outreach/Complex Care Service

This position involves providing Allied Health Assistant support to the HIV Outreach and Complex Care Service (HOCCS) within the Victorian HIV Service. The incumbent will work as part of a motivated team of HIV Specialist Nurses and Social Workers where responsibilities include the organisation and communication of a range of administrative and patient related activities. They will work closely with the interdisciplinary team to provide a flexible and responsive service that works to meet each client’s identified therapeutic goals.
The role will assist coordinating the HIV Assist Service and provide direct support work to clients in the HIV Assist. They will assist in coordinating Personal Care Workers working in HIV Assist and oversee their roster. The role will assist people living with HIV and complex care needs in the community maintain their health and engagement with health services and maintain their community based accommodation. This will include coordinating hotel accommodation for rural and regional clients of the service. The role will also engagement with clients from culturally and linguistically diverse (CALD) backgrounds of the program adjust to their HIV diagnosis and navigate health and immigration systems/services.
