Full time - 80 hours per fortnight
Registered Psychiatric Nurse Grade 3, Social Worker Grade 2 or Occupational Therapist Grade 2
Based at St Kilda Road Clinic
An exciting opportunity for a mental health clinician (RPN3, SW2 or OT2) to join the expanding Navigations Team at Alfred Community Psychiatry.
The role of the Primary Care Coordinator is to offer short-term support, linkage and referral to primary care community services for clients who would benefit from clinical support for a period of up to 3 months. The Primary Care Co-ordinator will work with the Navigations Consultant Psychiatrist to provide clinical care and coordination for clients.
In this role, you will;
Play a key role in developing, implementing and evaluating clinical interventions and capabilities
Support the philosophy of collaborative, coordinated and integrated care
Ensures that all clients referred for Primary care will collaborate in the development of an individualised care plan.
To be considered for this role you will have professional experience working in mental health settings and be a registered health professional - RPN, OT or Social Worker
