- Case Manager - Registered Psychiatric Nurse Grade 4
- Full time / Part time (minimum 4 days/week) permanent ongoing position
- Aged & Liaison Psychiatry Service located at Caulfield Hospital
- 0.8 to 1.0 EFT - Monday to Friday
As an integrated multidisciplinary team, MAPS provide crisis assessments and ongoing support for those over 65 in the Inner South catchment area. This position provides excellent opportunity to develop your assessment, risk management and decision making skills in a variety of areas of mental health including triage, crisis assessment work and case management.
Our clients present with a range of complex psychosocial and medical problems, requiring a creative and innovative approach to case management whilst still applying principles of evidence based practice. Our team comprises of medical staff, nursing, social work, occupational therapist and psychology, providing the opportunity for discipline specific intervention.
We have very manageable caseloads which is Alfred Health’s best kept secret.
Experience with older adults is desirable but not essential as MAPS has an extensive and well supported education program, committed to up skilling staff in all areas of our work.
