Patient and Family Services encompasses Social Work and Pastoral Care. The Social Work service extends across Aged Care and the Rehabilitation Units, including the Acquired Brain Injury Unit and the Independent Living Unit. There is an active Professional Development Program and strong professional links with other Allied Health professionals as well as with the Departments of Patient and Family Services at the Alfred and Sandringham Hospitals.
An exciting opportunity to work in a fast paced rehabilitation facility providing social work intervention and support to patients and families with a variety of complex psycho-social needs. An ability to work with new and developing service structures such as NDIS and My Aged Care and capacity to engage, support and provide advice to our consumers and interdisciplinary teams in relation to discharge planning pathways is essential. Strong, empathetic communication skills combined with the ability to support and counsel patients with complex psycho social needs are required in this role. Excellent supervision is provided and the role is located within a supportive team environment.
Knowledge of legislative frameworks and National Disability Insurance Scheme and My Aged Care.
Ability to mentor and train social work students
Ability to manage caseloads and provide outstanding patient care
Interest to participate in research opportunities within the Social Work Team
