Alfred Health is a leader in health care delivery, improvement, research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne, from ambulatory, to inpatient and home and community based services.

  • 80 hours per fortnight
  • Allied Health Grade 3 or Nursing Equivalent

The Role

We have an exciting opportunity for an experienced nurse or allied health clinician to join the Alfred Health Transition Care Program team as a Senior Clinician. In conjunction with interdisciplinary teams our aim is to improve the lives and outcomes for older Victorians. The Alfred Health Transition Care Program is a goal orientated, time limited program and consists of 88 Commonwealth and State jointly funded places which are divided between residential and community based programs. The unit reports to the Rehabilitation and Aged Care Program stream at Caulfield Hospital.

Skills and Responsibility

Demonstrated leadership, excellent communication and problem solving skills are vital for the service coordination of clients with diverse needs accessing the transition care services. You will be responsible for ensuring seamless access to the Alfred Health TCP by liaising with relevant health professionals, providers and other stakeholders.

Your key responsibilities will include:

  • Be able to work autonomously and independently and yet facilitate a collaborative approach.
  • Have demonstrated skills in priority setting and managing a complex workload.
  • Have and understand of, align with and role model the Alfred Health capabilities.
  • Demonstrate extensive knowledge and experience with aged clients in an acute/sub-acute, residential and community settings and with patient flow.
  • Have demonstrated experience with staff supervision.
  • Ensure that clinical practice meets the standards required under the Aged Care Act and National Transition Care Program guidelines.
  • Have well developed computer literacy skills
  • Hold a current Victorian driver's licence and a willingness to work across all sites.
